Completing a root cause investigation

To complete a root cause investigation with Mazlan:

1. Access Mazlan within your system.
2. Enter the Five Whys investigation prompt.

3. Answer each Why question from Mazlan to find the root cause.
4. Review the root cause Mazlan identifies from your answers.

4. Ask Mazlan to propose actions to address the root cause.
5. Review and select suitable actions to implement.
6. Save the investigation results and agreed actions with owners and deadlines.
