Raising an Incident from an Audit
There are two ways to create an Incident finding within an Audit record:
Create Incident record from the Findings Section
To create an Incident finding within an Audit record:
- Access an Audit record that is set with a status of "Performed"
- Navigate to the [Findings] section of the Audit record and click [Raise Non-Conformance]. This will open the "Audit Non-Conformance" window.
Note: Although the Audit Calendar value selected will be set to create Incident findings the icon will still reference a non-conformance; this is because, regardless of the method of raising a non-conformance (CA/PA or Incident), the raised finding is classed as a non-conformance for the basis of the Audit being performed.
- Within the "Describe Finding" window, enter the Event Title information into the relevant field
- Check one (or more) scope items that you intend to include as classifications within the Incident record. You can also add additional classifications from this section
- Click [Next]
- Within the "Risk Matrix Selection" window, select a valid matrix (if applicable) and click [Next]
- Within the "Perform Risk Assessment" window, set the likelihood and severity values and enter a justification if required. Click [Next]
- Within the "Confirm" window, populate the required fields
- Select [Finish] to complete the wizard. If you wish for the record to open upon completion, ensure that the "Open record on finish" checkbox is populated (this is populated by default)
An Incident record will now be added to the Audit record that you can populate and complete as normal.
Create Incident record from a checklist
To create an incident from a checklist:
- Access an Audit record that is set with a status of "Performed"
- Navigate to the [Checklists] section of the Audit record and click [Open Checklist]. This will open the "AuOpen Checklist" window.
- Select "Complete Checklist" and press [OK] to proceed
- Press [Start] to begin the checklist
- You can then Click on [Raise a New Non Conformance] from within any of the questions in your checklist.
