Performing a risk assessment on an Incident

One of the key parts of the incident module is being able to risk assess an incident risk record.
Risk assessing an incident can be done following the investigation of an Occurrence report or while managing a finding raised from an Audit. This can be done in Windows only.
1. Perform the Risk Assessment

Click the Edit button on the Incident record.
- Go to the Risk Assessment tab.
- Click the New Risk Assessment button.
- In the Risk Assessment pane complete the details required.
- Click OK.

To perform a risk assessment on an incident record:
- Open the Risk Assessment section of the Incident and click the plus icon.
- From the drop-down, select the risk matrix that you wish to use when performing the risk assessment. For details on building risk matrices, please refer to How to Build a Risk Matrix in Ideagen Quality Management.
- The risk assessment screen will open, you can enter the following information:
- Reason: This will become the title of the risk assessment.
- Comment / Justification: These are free text fields - a comment on the risk assessment and a justification of the risk assessment.
- Owner: The person who is responsible for this risk assessment.
- Performed On: The date that the risk assessment was performed on.
- For each axis in the matrix, choose a suitable risk value. Hovering over any value will give you additional information to help you select the correct value. The Risk Matrix preview will show you the outcome of the risk assessment, based on the values you have chosen. Based on the outcome, a suggested action (if one has been defined when creating the risk matrix) will be shown at the bottom-left of the screen.
- Click [OK] to save. The risk assessment will then be shown on the incident record, together with the title, performed date, suggested action, result (including colour) and the matrix used.
- You can edit any existing risk assessment by double-clicking its name, unless the incident is closed. If you wish to edit or perform a risk assessment against a closed incident, you will need to re-open it first.
