To configure risk matrices:

1. Build a New Risk Matrix
2. Add a Matrix to an Incident Template

Building a New Risk Matrix

1. Launch Ideagen Quality Management 
2. Open the Incidents Module
3. Click on [Edit], then [Risk Matrices]
4. Click [File], then [New Matrix] to launch the Risk Matrix creator window.

• A Name and a Description 
• A Likelihood and Severity rating
• Up to 10 extra severity perspectives (e.g. environment, reputation, financial)

5. Enter a Name, Description and label your X &Y Axis. You can then add in any Perspectives of severity as below

6. Enter the Decision Aids that you will use for your X & Y Axis as below

7. You can then configure your Bandings with Labels, Actions and Colours as below.

8. Your completed Risk Matrix will then look as below. 

1. Launch the Incident Module
2. Click on [Edit], then [Incident Templates]
3. Double Click on the desired Template.
4. Click the Risk Assessment tab
5. Click the [Add] icon to add a risk assessment
